Purpose of General Appeals Process Essay

The purpose of a General Appeals Process is used to challenge payer’s decision to reduce, deny or to downcode any claim. The provider however, can ask for a review of the payer’s decision. The patient also has a right to ask for a request of the appeal. The claimant or appellant is the one that is filing the appeal. That could be an individual who is the provider or the patient. The basic steps are simple of the appeal. This is where the payer has a consistent procedure to handle the appeal. This has to do with what kind of appeal it is. The practice staff does a review on the procedure before other actions are taken. The staff then takes the necessary steps to move forward. The appeals must be filed by a certain time after the determination. The payers must go through three steps. These steps are a complaint, an appeal and the grievance. There is not a bunch of time to spend on this procedure. After this is done and the payer is still denying the claim, the claimant can take another step. This can be reviewed through the state insurance commissions since most payers are licensed through them. The state insurance commissions must have all copies of the file and to see why the claim was denied. The reason that the claim was denied to could be simple reasons. The reasons are mostly human error. The most common claim denials are due to not using the patient’s health care identification number and the correct name on the card. The other reasoning could be not putting down where the services were done. The last but not least could be invalid diagnosis codes. This could all be why the claim is denied. Stress is a major contributor to burnout in the Law Enforcement field. There can be many contributors to this, but among them are the idea of having to use deadly force against someone or witnessing deadly force being used against a partner or coworker, lack of support from department members and leaders, having family life and traditions disrupted and so forth. Law Enforcement work is not something that can be easily dealt with and easily overcome. An officer has to deal constantly with dangers that aren’t normally faced by other lines of work or other career-paths. Police constantly deal with the dregs of society and because of this, they are forced into situations that aren’t conducive to an easy work and home life. Many resources exist to help officers deal with this stress. Some of these resources can be a strong faith and trust in one’s religion and religious leaders. A strong support system within an officer’s respective department, counseling offered and available to officers once they do undergo a stressful encounter or situation. Such counseling is helpful, but is often placed by the wayside by officers because they feel they are strong enough to handle such situations themselves, without help! It is important for an officer to realize that help, in such situations, is not a bad thing and will not cause others to look down on them. The strongest man (or woman) is the one who realizes that he cannot handle the situation alone and asks for help. An officer needn’t feel he is above asking for help. Often the first thing that will help an officer down the path of healing is that first initial instance when he/she admits to a superior, â€Å"I need help with this!†

Important People of History Essay Ancient Greece was known for many things, including great thinkers and philosophers. Many names come to mind from this period, including that of Socrates. Certainly, none are so colorful. He wrote nothing, but his impact was unmatched by others. His ideas were revolutionary, and caused enough controversy that he was finally put on trial in Athens, guaranteeing that his mark on history would be indelible. Socrates was born in 469 B. C. E. to Sophroniscus and Phaenarete. Little is known about him beyond what others wrote, since he resisted any urges to put words on paper. What is known about him comes from Aristophanes, Xenophon, and Plato, all of whom wrote about him. However, their narratives differ in some crucial ways, and so there are some things that will never be known about the enigmatic philosopher. Socrates was a firm believer in the power of the intellect. He insisted that â€Å"a man must order his life by the guidance of his own intellect,† believing that it was this above all that must be cultivated. He never hesitated in saying what he believed, and this tendency made him unpopular in many circles. However, he did not allow this to deter him from expressing these beliefs. Knowledge and truth were paramount to him, and he would not sacrifice these virtues for anything. When he was eighteen, Socrates was required to do the duty of every citizen, and this included training in the Athenian militia. He served the years that were expected of him, engaging in battle in 432 as a foot soldier to help put down a revolt. On the way home from the siege that followed, Socrates saved the life of Alcibiades during a subsequent battle near Spartolus, losing the battle. They returned to Athens in 429, where he was accused of helping the playwright Euripides to write his tragedies. Important People of History Page 2 of 5 Socrates next saw battle at Delium in 424. The Athenians saw defeat yet again, although Socrates distinguished himself once more. In another year, he was yet again immersed in battle, this time at Amphipolis. This ended in another defeat, and was also the last battle that Socrates fought. Peace was made between Athens and Sparta, and this is when Socrates could begin to concentrate in earnest on debates and discussions to pass on his ideas and beliefs. Education, democracy, and Mystery religion were key themes in this. During this time, Socrates married Xanthippe, and they would have two sons. Socrates perhaps became best known for the Socratic Method. This method was meant to help intellectuals come to correct conclusions, and it made definition the most essential point of this. In a desire to find the truth, Socrates believed that one must â€Å"require a standard of truth and an explanation of the causes of error. † The comparison of two ideas is necessary in making a judgment, and those ideas must be clear and not vague. This method lasted throughout the centuries, and is still practiced today. Socrates is also known as the founder of utilitarianism. Ethics and the process of applying his methods to them was a key interest of his, and this study led to utilitarian ideas. With this, he came to the conclusion that good is useful and â€Å"virtue is happiness. † This formed the core of his paradox, which is that men do not willingly commit wrongs, and that this is done only through ignorance. While Socrates wrote nothing, he spent a great deal of his life conversing with others. This was a major part of his Socratic method. He never attempted to teach, for he believed that he had no knowledge. He professed that he was only exceptional in the fact Important People of History Page 3 of 5 that he knew that he knew nothing. It was a goal of his to prove that popular ideas were inconsistent, and he wished to convince all to whom he spoke that their convictions could not stand up under examination. Socrates was well known for frequenting places such as marketplaces, where he would engage people in conversation. He enjoyed doing this with people of all ranks and stations, often putting off important people and gaining their animosity. His manner and lack of deference gained him many enemies; many resenting what they felt were his ridicule. There was nothing that Socrates felt was too sacred to question and criticize, and this included both religion and government. He was perhaps more agnostic than anything, and he had no compunctions about questioning and criticizing the gods. This added to feelings of animosity against him, as did the fact that he was also quite vocal against the ruling powers. It was this that was to lead to his arrest and subsequent trial, which is perhaps the pinnacle of his career. Socrates was arrested when he was seventy on charges that â€Å"he does not believe in the gods recognized by the city, but introduces strange supernatural beings; he is also guilty, because he corrupts the youth. † The political reasons were kept quiet, instead focusing on impiety, and the sentence proposed was that of death, although those who accused him had no real desire to put him to death, only to force him to leave and no longer be a thorn in their sides. However, Socrates refused to leave, instead remaining behind to face the charges against him. A life away from his interactions with the Athenians would not have suited him, and so he remained. Important People of History Page 4 of 5 Socrates faced a jury of 501 men. This jury, as all Athenian juries, would vote twice. Their first vote was on guilt or innocence, and their second vote was for the sentence. While Socrates expected that he would be convicted, he expressed surprise at â€Å"how closely the jury was split on the first and basic question. † It was a difference of six percent that would have turned the tide, acquitting him. Socrates had the chance to avoid a sentence of death once the verdict came in. It was a practice of the Athenian law system to allow the defendant to put forth another punishment, and the jury would choose between the two. However, the punishment that Socrates suggested was merely a small fine, which was so minuscule a punishment that the jury had little choice but to condemn him to death. It was not for thirty days after his trial that Socrates would meet his end. During that time, his friend Crito appealed to him to escape, but Socrates believed that the harm that would come from that would be a greater harm to the city than to allow himself to follow through with the sentence, and that it would have â€Å"brought shame on his family and friends. † Socrates did not seem to fear his death. In fact, on the day upon which he was scheduled to die, he seemed quite happy and pleasant. Death was perhaps another opportunity for him to learn and experience. He was also not alone when the end came, as his wife and younger son, as well as friends, were present. He spent his last day doing what he loved best, discussing philosophy and once more passing on his wisdom. Xanthippe was taken home before his actual death took place, leaving him with his Important People of History Page 5 of 5 friends and advising them to agree if they believed what he said was truth, and to oppose it if they did not, refusing to restrain themselves. To the end, he was determined to hold onto his ideals and beliefs, and insisted on others doing so as well. After bathing and then meeting with his family once more, he met with his executioners. He was given a glass of hemlock after having its effects explained to him. After this formality, he â€Å"cheerfully took the cup and drank. † Thus ended the life of one of the most profound leaders of philosophy. Socrates, through all of his life, lived up to the ideals and beliefs that he taught. He never compromised these, even up to the day that he died. To him, the importance of remaining true to himself and his teachings was paramount, and to do otherwise would be an injustice. He did not simply talk and hope that others learned from him; he lived by example and died by the same. His legacy proved to be quite enduring, and is still a cornerstone of philosophy to this day. His students, such as Plato, helped to preserve this and pass it on. Without Plato and others, little would be known today of Socrates. The impression he made was a lasting one, and a good example even today, encouraging people not to be afraid to question and discuss, because without these things, learning cannot be successful. Bio-electricity is an interdisciplinary theme which refers to the production of electric, magnetic and electromagnetic fields in biological tissue (Cameron, 1996). This essay focus on the production of electric fields for use in organisms rather than in electrical circuits. An electric field is an area around a charged particle created by an electric force; the magnitude and direction of this force is determined by the charge of the particles surrounding it (Dictionary, no date). An example of this in Biology is the electrical system of the heart and the maintaining of membrane potentials in cells in the body. Bio-electricity in Nature American Paddlefish An example of an organism which uses Bio-electricity in an enhanced form is the American Paddlefish (or Spoonbill Catfish). These are large, freshwater fish from North America which have an extensive, paddle-like flat nose. The extended nose of the fish is embedded with pores with cells called electroreceptors (Thomas, 2016). These pores spread over the fishs body, from the nose to gill covers, covering around half of its skin (Wilkens, 2001). This allows the detection of weak electric currents in the water they travel in, thus allowing them to hunt their main source of food; plankton. These sensors are extremely sensitive, and are able to respond to electrical fields of less than 1/100th of one 1-millionth volt per centimetre (Wilkens, 2001). All organisms emit weak electric signals (e.g. electrical impulses due to the beating of the heart), and thus can be detected by these cells. The electrical signal is sent to the brain via the neurons. Paddlefish being able to detect these fields aid their survival, as their environment is generally very dark (Thomas, 2016). Electric Eel Another example of Bio-electricity in nature being used in a different way is with the Electric Eel. It can do this due to the electrogenic cells in its body called electrocytes, each with a negative charge of around 100 millivolts (Scientific American, 2005). These are all modified muscle cells in the eels tail. When a signal from a command nucleus arrives to each of the cells, a neurotransmitter is released. These are chemical signals which allow information to be transmitted between nerve cells (Cherry, 2016). As a result of this, a path of low resistance is created, connecting the interior and exterior of each cell. This allows the cells to behave like batteries, with opposing sides of the cell acting as positive and negative terminals (Scientific American, 2005). This is used to both hunt and kill prey. If in sight, prey can be hunted by the eels the release of high voltage pulses of a frequency of 400/s. If these pulses reach another organism, it will interfere with its neurons, causing them to involuntarily contract. This contraction is extremely strong due to the sequence of the pulses released; each lengthy torrent is succeeded by a pair of rapid pulses, making the process as efficient as possible. As a result of this, the prey can experience complete short term paralysis, allowing the eel to attack. The effect is similar of that of a taser on a human (Yong, 2014). Similar to that of the Paddlefish, potential prey can also be detected through the use of Bio-electricity. The Electric Eel can send out pulses, again causing the prey to contract involuntarily. The resulting ripples in the water would reveal the location of any organism affected, as the eel is extremely sensitive to movement (it can strike within 20 thousandths of a second from when movement is detected). This provides a further opportunity for the eel to kill (Yong, 2014). Moreover, the eel can kill prey (or deter predators) by directly shocking them. Though each individual electrocyte can create only a small electric field, the cells are connected as parallel batteries are in a circuit, creating a larger electric field. The combined electrocytes create 3 large electric organs which can take up 80% of the eels body (Giardina, 2016). This means an average Electric Eel can produce electricity of around 600 volts at 2 milliseconds at a time. Bio-electricity in Medicine The use of Bio-electricity to treat human conditions is being increasingly explored. A link between the immune system, the series of processes which works to primarily fight off disease, and the nervous system, processes which involve electrical signals within the body, is being researched. One of the main method of using our knowledge of Bio-electricity in Medicine is through the stimulation of the Vagus nerve, also known as Cranial Nerve (X). This is one of 12 cranial nerves, which are nerves which begin from the brain stem rather than the spinal cord. These nerves are responsible for the motor and sensory functions of our body (InnovateUs Inc, 2013). Of all the cranial nerves, the Vagus nerve is the longest, extending through the thorax, heart, stomach, abdominal muscles and others. This links the brainstem to the body and major organs (particularly the heart), allowing information to be easily transferred in an electrical circuit-like system (InnovateUs Inc, 2013). One of the uses of the Vagus nerve stimulation is the controlling of seizures in epilepsy patients. This is by connecting a small device to the left Vagus nerve in the neck. The device can then send out small electrical impulses via the nerve to the brain. This reduces activity in the brain which can lead to seizures; seizures themselves are caused by abnormal electrical activity in the brain. (Epilepsy Society, 2016). A second use of Bio-electricity in Medicine is the use of cardiac pacemakers in the treatment of Parkinsons disease, a neurodegenerative disorder caused by a loss of dopamine producing nerve cells in the brain (NHS Choices, 2016). To help reduce the symptoms of Parkinsons, which include rigidity, tremors and bradykinesia, doctors are implanting a small electronic device called a neurostimulator in patients (Lazarus, 2014). The device sends out regular electrical impulses to areas of the brain responsible for movement. This allows impulse to block the irregular signals which cause the shaking in patients, giving them more control. The entire process is called Deep Brain Stimulation and is a widely used technique to help improve quality of life in people with Parkinsons. Conclusion In conclusion, Bio-electricity is a naturally occurring phenomenon occurring in humans and animals alike, and is essential to everyday living. Its natural uses explored include communication between the brain and body via neurotransmittors and nerve cells, the detection of prey and predator in animals and its use in hunting. With modern technological advances, its use is being extended to the treatment of disorders in humans including depression, Parkinsons, arthritis and epilepsy, many of which through the installation of electrical devices which send out impulses. This encourages the team members to work as a team since if it is a team effort the bonus will be higher if compared to an individual payout. The only differences between ABC Managed Services and the other 2 organisation (ZX Services and MY Agency are both locally owned and not foreign owned like ABC Managed Services and been in the industry for more than 2 decades. All of this 3 organisation have a pay strategy to support their business strategy. Why? In my opinion, I dont think there is one business strategy which seems to fit for any organisation. Most organisations will likely have the mixture or combination of cost cutter, customer centered or innovator as their strategy. In ABC Managed Services, their business strategy are based on being innovative, customer oriented since its in the services industry and always looking towards cost cutting measures and increase productivity. With these business strategies, it becomes the main drive for them to achieve their vision to become a leader in the HRO/ITO industry worldwide and remain excellent in customer satisfaction. The reason why I choose the following format is because the performance appraisal form should be simple and easy to understand by employee and the supervisors. This method used to describe a performance rating that focused on specific behaviors or sets as indicators of effective or ineffective performance. This method is very useful and exact. This will also allow supervisor just focus on the questions based on competencies which had been stated on the form and provide rating according to employees performance. The appraisal form will also wont take much time for the employee to prepare and we used simple English as mode of communication.
